Mental Health Practitioners support adults whose needs cannot be met by local talking therapies but who might not need ongoing care from hospital or mental health teams. They can refer patients to a range of different services.
Our team also includes: Advanced Nurse Practitioner, a Practice Nurse, a Paramedic, a Community Pharmacist, a Clinical Pharmacist, a First Contact Physiotherapist, Social Prescriber, Health Care Assistant and GPs.
